Refills of your prescriptions

Effective July 1, 2017, refill requests must now come directly from your pharmacy. Please call your pharmacy and ask that they submit an electronic request. Be sure to allow 48 hours for your request to be processed. Contact your pharmacy early in the day, and early in the week. Please note that requests received after 3:00pm will not be addressed until the next business day, and may take up to 48 hours for completion. Read more about our refills policy >

If you require a refill of a controlled substance, gather the required information below:

 

  • Patient Full Name

  • Patient Date of Birth

  • Primary Provider/Practitioner at CMMD & Associates

  • Drug Name (from the bottle, please)

  • Dose (from the bottle, please)

  • Times taken per day (from the bottle, please)

  • Pharmacy Name, Street Address, Phone

  • Does your Insurance allow 90-day supplies?

  • Is this refill needed urgently?

Please note that we are unable to call in or authorize refills of controlled medications after hours or on weekends. Due to tightened DEA regulations, medications such as narcotics, anti-anxiety medications, and ADD medicines will only be refilled Monday-Friday, 9 am to 5 pm. Please check your medication quantities carefully before the end of the week. Please note that if you are on a controlled substance for ADD (Adderall, Ritalin, Vyvanse, Concerta, Focalin and others) you MUST be seen at least every THREE months in order for refills to be authorized.

Referrals to specialists

Please note it is our policy that patients are current on their PCP follow-up visits before we can renew any specialist referrals. In general, this means you must have been seen in our office in the past 3 months for your medical condition. For new medical concerns, it is our policy that patients see their Primary Care Physician to determine if further evaluation by a specialist is needed before issuing a new referral. We do not routinely refer to chiropractors. You must discuss any referral to a chiropractor with your Primary Care Physician.

 

To request a referral please provide the following:

 

  • Patient Full Name

  • Patient Date of Birth

  • Patient Contact Number

  • Primary Insurance

  • Specialist Name

  • Specialist NPI Number

  • Location (specific office address) for the Specialist you are seeing

  • Diagnosis code (reason you are seeing this specialist)

  • Is your visit related to a new medical condition?

  • Date of Specialist Appointment

  • Date of last PCP Appointment

Lab Orders/Blood Work

Effective August 1, 2016, CMMD & Associates no longer schedules appointments for blood draws only. We will draw blood as necessary if patients are in the office for a visit; we just can't do it separately.

We prefer to wait until you arrive for your visit to order lab work. This is so that the proper tests are ordered after your provider's assessment. In the past, we have found that patients who have had their lab work completed prior to a visit, were often frustrated to learn they need more testing based on their clinical findings. To help prevent two trips to the lab, we encourage all patients to wait until their appointment to request lab orders. If you have a specific question or concern, email your provider directly. 

Lab orders are now submitted to all Penn Medicine sites, Quest, and LabCorp electronically. Upon arrival at your lab, the technician will be able to retrieve the electronic order.  Please note that due to privacy laws, we are no longer able to email lab orders.
